1. Assumption of Risk

I understand that participation in activities at AlJoh's involves swinging golf clubs, hitting golf balls, and being in proximity to others doing the same. These activities carry inherent risks of injury, including but not limited to:

• Strains, sprains, or impact injuries

• Being struck by a golf ball or club

• Slips, trips, or falls on the premises

I voluntarily assume all risks associated with participating in these activities.

2. Release of Liability

In consideration for being permitted to use the golf simulator facility, I agree to release, waive, and discharge AlJoh's, its owners, employees, contractors, and agents from any and all claims, demands, or causes of action arising from injury, damage, or loss, whether caused by negligence or otherwise, to the fullest extent permitted by law.

3. Damage Responsibility Clause

I acknowledge that I am financially responsible for any damage I cause to equipment, facilities, or property due to misuse, negligence, or failure to follow instructions, and I agree to pay the full cost of repair or replacement at the discretion of the facility.

4. Acknowledgment

I have read this Liability Waiver and Release Agreement and fully understand its terms. I sign it freely and voluntarily with the intent to be legally bound. If signing on behalf of a minor, I certify that I am their parent or legal guardian and assume all risks and responsibilities for their participation. I acknowledge by signing below, I accept all responsibility for myself and my party while on Aljoh's premises.
